Educational Requirements:
- High School Diploma or GED required
- Bachelor's degree preferred.
- Minimum of 1 year recent experience working in a health care field, engaging with patients and/or experience in care coordination/case management, medical office, community outreach, social work or customer service.
- Basic knowledge of community resources in Los Angeles, and familiarization with community-based health care delivery.
- Demonstrated combination of training experience, problem solving skills and background that involves customer service.
- You must be comfortable and familiar with working with diverse populations, especially underserved members of the community including those suffering from mental health disorders and substance abuse.
- Ability to convey and/or receive written/verbal information to/from various audiences in different formats.
- Basic computer proficiency
- May require standing, walking, sitting for extended periods of time
